Our approach, informed by the community and our community partners, builds on more than two decades of SUHI’s experience developing, training, supporting, and sustaining the CHW role, a critical resource in value-based health care to promote health equity in overburdened and under-resourced communities. CROWD’s training model is enhanced by a diverse staff including CHWs, administrators, evaluators, and community health, learning development, and training specialists. We work together to deliver a collaborative and meaningful curriculum to all our participants.
